This Telehealth Informed Consent describes the nature of telehealth services offered through the SengMed platform and the rights and responsibilities involved. Please read it carefully before requesting a consultation.
This document is a general template provided for the SengMed website and should be reviewed and finalized by qualified legal counsel and clinical leadership before publication.
1. What Telehealth Is
Telehealth is the delivery of healthcare services using secure electronic communications, such as questionnaires, messaging, and audio or video consultations, between you and an MOH-licensed doctor who is not in the same physical location. SengMed’s telehealth services are intended for patients located in Singapore.
2. Nature of the Services
Through SengMed you may complete an online health assessment that is reviewed by an independent doctor registered with the Singapore Medical Council. The doctor determines whether treatment, including a GLP-1 medication, is clinically appropriate. Prescriptions are issued solely at the doctor’s discretion and only when appropriate.
3. Benefits and Limitations
Telehealth can offer greater convenience and access to care. However, it has limitations: a doctor cannot perform a hands-on physical examination, and some conditions cannot be assessed or treated remotely. Your doctor may determine that an in-person consultation or additional testing is needed.
4. Risks
Possible risks include technology failures or delays, limited information affecting clinical decisions, and, rarely, security breaches despite reasonable safeguards.
5. Medication Information
Some medications may not be registered with the Health Sciences Authority (HSA) for weight management. Where this applies, your doctor will discuss it with you before prescribing. Discuss any questions or side effects with your doctor, and seek emergency care for urgent or severe symptoms.
6. Your Responsibilities
You agree to provide accurate, complete health information, to follow your doctor’s instructions, and to inform your doctor of any changes in your health or any adverse reactions.
7. Privacy
Information shared during telehealth is protected as described in our Privacy Policy and is handled in accordance with Singapore’s Personal Data Protection Act 2012 and applicable Ministry of Health guidelines.
8. Right to Withdraw
Your participation is voluntary, and you may withdraw your consent or discontinue telehealth services at any time without affecting your right to future care.
9. Emergencies
Telehealth is not for medical emergencies. If you are experiencing an emergency, call 995 for an ambulance or go to the nearest hospital Accident & Emergency department.
10. Acknowledgement
By using the Services and requesting a consultation, you acknowledge that you have read and understood this consent and agree to receive telehealth services through the SengMed platform.
