This Telehealth Informed Consent describes the nature of telehealth services offered through the SengMed platform and the rights and responsibilities involved. Please read it carefully before requesting a consultation.
This document is a general template provided for the SengMed website and should be reviewed and finalized by qualified legal counsel and clinical leadership before publication.
1. What Telehealth Is
Telehealth is the delivery of healthcare services using secure electronic communications, such as questionnaires, messaging, and audio or video visits, between you and a licensed provider who is not in the same physical location.
2. Nature of the Services
Through SengMed you may complete an online health evaluation that is reviewed by an independent, licensed provider. The provider determines whether treatment, including a GLP-1 medication, is clinically appropriate. Prescriptions are issued solely at the provider’s discretion and only when appropriate.
3. Benefits and Limitations
Telehealth can offer greater convenience and access to care. However, it has limitations: a provider cannot perform a hands-on physical examination, and some conditions cannot be evaluated or treated remotely. Your provider may determine that an in-person visit or additional testing is needed.
4. Risks
Possible risks include technology failures or delays, limited information affecting clinical decisions, and, rarely, security breaches despite reasonable safeguards.
5. Medication Information
Some medications offered may be compounded. Compounded medications are not reviewed by the U.S. Food and Drug Administration for safety, effectiveness, or quality. Discuss any questions or side effects with your provider. Seek emergency care for urgent or severe symptoms.
6. Your Responsibilities
You agree to provide accurate, complete health information, to follow your provider’s instructions, and to inform your provider of any changes in your health or any adverse reactions.
7. Privacy
Information shared during telehealth is protected as described in our Privacy Policy and the treating provider’s Notice of Privacy Practices.
8. Right to Withdraw
Your participation is voluntary, and you may withdraw your consent or discontinue telehealth services at any time without affecting your right to future care.
9. Emergencies
Telehealth is not for medical emergencies. If you are experiencing an emergency, call 911 or go to the nearest emergency room.
10. Acknowledgement
By using the Services and requesting a consultation, you acknowledge that you have read and understood this consent and agree to receive telehealth services through the SengMed platform.
