The key difference: one hormone or two
This is the heart of it:
- Semaglutide is a single-action medication. It mimics one gut hormone, GLP-1, which curbs appetite and increases fullness. Brand names include Ozempic and Wegovy.
- Tirzepatide is a dual-action medication. It mimics GLP-1 and a second hormone, GIP, which also influences appetite and how the body handles sugar and fat. The brand name is Mounjaro.
Because tirzepatide works on two pathways instead of one, it is often described as the more potent option.
Expected results
In clinical studies, both produce significant weight loss. On average, tirzepatide has tended to produce somewhat greater weight loss than semaglutide, though individual results vary widely. Factors like your starting weight, dose, consistency, diet, and activity all matter, often more than the choice of medication itself.
Dosing
Both are usually given as a once-weekly injection with a pre-filled pen, and both are started at a low dose that is increased gradually over several weeks. This “titration” helps your body adjust and keeps side effects milder. Semaglutide is also available as a daily tablet in some cases.
Side effects
Both medications have a similar side-effect profile. The most common are digestive, mild nausea, constipation, or reduced appetite, and these are usually most noticeable when starting or increasing the dose, then ease over time. Neither is clearly “gentler” than the other for everyone; tolerance is individual.
